    Before you complete this application please ensure you have the following information ready:

    • Read attached Terms and Conditions carefully
    • Equine must be in the correct ownership.
    • Equine passport (breeding, studbook, UELN)
    • A screenshot of your Equine Operator Profile (ROI) or Equine Establishment Number (NI)
    • Proposed veterinary practice details
    • Registered owners Bank Details (IBAN and Name on Bank Account)

    PLEASE NOTE: To ensure that schemes operate on a first come first served basis; applications where the above is not in order cannot be approved. Should the applicant update their/horse’s details thereafter, a new application will need to be submitted.

  • I hereby warrant, represent and agree as follows:

    1.   The particulars I submit are true and accurate in every respect. In the event of any becoming inaccurate updated revised particulars will be furnished immediately in writing to Horse Sport Ireland (“HSI”).

    2.   I am not aware of any matter or circumstance that might affect or be relevant to the testing of my 2026 equine’s for EVA, EIA or CEM  or ultrasounds that I have not disclosed in writing to HSI.

    3.   Any details/materials which may be furnished by me with the application are solely owned by me (or where I apply as agent, by the owner(s)) and no third-party consent or authority is required for the use thereof by HSI.

    4.   I confirm that am the registered owner horse(s) subject to the application and and reference that this ownership statement is made in compliance with SI 201 of 2016 and a false, misleading or incomplete declaration is liable to the penal provisions of the Animal Health and Welfare Act 2013. If there are any agreements with third parties, HSI must be made aware of this prior to the screening of the horse(s) under this scheme.

    5.   All equines included in the scheme must be active breeding animals, at stud in Ireland and available for breeding in 2026.

    6.   I agree to indemnify HSI on demand in full against any loss, claim or damage arising in the event of any breach of the foregoing warranties or any of them.

     

    I hereby confirm, agree and acknowledge as follows:

    1. HSI are hereby irrevocably authorised to screen my horse for EIA, EVA and CEM  and Ultrasounds through a suitable qualified veterinarian and for sample testing at an ISO 17025 accredited laboratory. The applicant must return these results back to HSI.
    2. HSI will publish results in an anonymised manner as they may deem fit, Horse Sport Ireland publications and website or otherwise and to provide the result of EIA, EVA, CEM of your horse to the relevant studbook (including in cases where they are not registered in HSI studbooks) and any other information furnished above or pursuant to this application, without being under any obligation to do so.
    3. Without prejudice to the generality of the foregoing I agree to be bound by the result provided by the testing laboratory referred to above in the event of any dispute arising in relation to this application or any selection or classification pursuant thereto.
    4. I understand that HSI, or my horse's studbook of origin, may use the laboratory results for bona fide studbook administration and national breed improvement purposes associated with Horse Sport Ireland.
    5. In the event of the equine being sold or otherwise disposed of to procure that the purchaser/acquirer is bound by the terms hereof insofar as any obligation herein is continuing or un-discharged.
    6. (In the case of multiple owners) each of the above warranties, representations and agreements are deemed to be made by or on behalf of each owner on a joint basis.
    7. By completing this form, I agree to provide Horse Sport Ireland with access to the information I have supplied in this form. This is in accordance with HSI's Data Protection and Privacy Policies (see below). Each applicant must review the data processing information contained within the scheme application form and confirm that they understand how their data will be processed.
    8. I confirm I have read, understood and agree to the terms of this scheme and how my data will be processed, shared, stored and retained.

     

    By completing an online application, the applicant declares that:

    • I understand and agree to the terms and conditions of this scheme in their totality.
    • I provide my informed consent to participate in this scheme.
    • I understand and provide consent for my equine’s data to be used for classification and studbook purposes, including but not limited to publication.
    • I understand and provide consent for my equines’ veterinary data to be used for research purposes, including but not limited to scientific publication.
    • I understand that a positive for a notifiable disease is required by law to be notified to the relevant authorities.
    • I understand that HSI will publish a list of awardees (forename, surname, county and equine name) on the HSI website.
    • I understand that drawdown of funding is subject to the return of relevant documents and completion of scheme feedback.
