    Funded by the Department of Agriculture, Food and the Marine under National Breeding Services and delivered by Horse Sport Ireland.

    • Equine must be in the correct ownership.
    • Equine passport
    • Accurate dam & sire information
    • Recipient mare passport details
    • Relevant filly/damline performance information
    • Registered owners Bank Details (IBAN and Name on Bank Account)

    This scheme is open to all equine’s resident on the island of Ireland. The applicant must be the registered owner of the mare subject to this application. This scheme operates on a first come first served basis up to the maximum allocation facilitated by the existing budget allocation. Applicants may apply for up to two equines under ET. Please read the scheme terms & conditions for performance criteria and eligibility before completing this form.

    Please Note: To ensure that schemes operate on a first come first served basis; applications where the above is not in order cannot be approved. Should the applicant update their/horse’s details thereafter, a new application will need to be submitted.

  • Applicants who fail to provide the following will not be eligible for funding.

     

    CHECKLIST for Embryo Transfer

    Please ensure that the veterinary invoice includes the following information:

    • Name and address of practice
    • Name and address of applicant
    • Correct amount on invoice in euro
    • Name and UELN of horse
    • Invoice is numbered, dated and marked paid

    45 Day scan certificate
    Zootechnical Certificate 

    IBAN provided for scheme payment MUST be the IBAN of the applicant.

    SNP genotype – if not existing in HSI

    Equine Operator Profile is required to be uploaded for all scheme applicants

  • By signing this form, I declare that:

    • I understand and agree to the terms and conditions of this scheme in their totality
    • I provide my informed consent to participate in this scheme
    • I understand HSI retain ownership of all data accrued under this scheme
    • I understand and provide consent for my equines’ anonymised data to be used for research purposes, including but not limited to scientific publication
    • I understand that HSI will publish a list of awardees (forename, surname, county and mare name) on the HSI website
    • I understand that drawdown of funding is subject to the completion of scheme feedback.
    • I have completed this form correctly and accurately. 

    1. The foregoing particulars are true and accurate in every respect. In the event of any particulars becoming inaccurate updated particulars must be furnished immediately in writing to Horse Sport Ireland (“HSI”).

    2. I am not aware of any matter or circumstance which might affect or be relevant to the application of the above-named mare(s) to this Scheme which I have not disclosed in writing to HSI.

    3. I agree to indemnify HSI on demand in full against any loss, claim or damage arising in the event of any breach of the foregoing warranties or any of them.

    1. I confirm that I agree to share my data with Horse Sport Ireland and my selected preferred veterinary clinic.

    2. I confirm I have read the “Terms and Conditions of the Embryo Transfer Scheme 2023” and I agree to accept and abide by them and any amendments that may be made by HSI thereto from time to time.

    3. In the case of multiple owners, each of the above warranties representations and agreements are deemed to be made by or on behalf of each owner on a joint and several basis.

    4. I confirm that I would like to apply for the HSI Embryo Transfer Scheme and give my permission for HSI to use my [horse(s)] data for research purposes as agreed in the terms and conditions.
