This form allows equine owners to submit copies of the horse’s passport pages where the food chain status (stamp) has changed.
The studbook/PIO must be notified of these changes so the Department of Agriculture’s AIM system can be updated. If a stamp has been applied to the passport and the PIO has not been informed, the animal may be stopped at the port and denied onward travel or transportation, as the passport will not match the offcial digital record.
The PIO/studbook will always check for these updates where the passport has come in for other services. However the stamp may have been applied more recently by the vet and therefore not yet notified to HSI.
It is the responsibility of the animal’s owner/keeper to ensure the horse’s passport is correctly maintained.

- Picture/copy in your phone/on your computer of the medical page in the passport where the stamp and the Horse's UELN can be clearly seen.
