Full pedigree and performance details must be provided on the form provided for horses ONLY. At the qualifiers, horse foal pedigrees will be assessed on the basis of the verified information provided on this application form to HSI. At least three generations of pedigree should be included. Please give as much detail as possible on relatives’ performance records, additional pages and documents from the relevant national (or international) sporting body verifying the performance should be included with the application and must be provided to ensure that appropriate points are awarded to your entry. HSI will not be responsible in instances where supporting documentation is not supplied.
Please note: Only pedigree and performance information supplied by the entrant will be included in assessment of the damline pedigree score. The onus is on the entrant to supply full performance details for the damline (dam, maternal granddam or maternal great granddam) to HSI. Only the information when verified will be included in the calculations of the pedigree score for the qualifiers.

I understand that pursuant to the Data Protection Act 2018 and the General Data Protection Regulation (GDPR) 2016, my consent is required for Horse Sport Ireland (HSI) to process personal data which it may have in its possession concerning me including disclosure to third parties such as, but not limited to, the Department of Agriculture, Food and the Marine. I note that HSI are responsible for administration services relating to the Irish Sport Horse, Irish Sport Pony and Irish Draught Horse Studbooks. I confirm that I have reviewed HSI’s Data Protection and Privacy policies (available at www.horsesportireland.ie or on request) and I note that HSI will only process my personal data for legitimate business purposes relating to the Irish Horse Register (IHR) studbooks, studbook research, stallion promotion and/or where required by law. All processing will have due regard to the DPA 2018 and GDPR 2016. I note that this personal data may include sensitive personal data, as defined within the DPA and GDPR, the processing of which requires my explicit consent. I note that HSI will honour my rights and freedoms at all times, including my rights as a data subject, as long as there is no other overriding law which limits those privileges. I consent to the processing of any information relating to me, either contained in this form or otherwise, for the purpose of administration services relating to the IHR.
I accept that at the HSI Foal Championship photographs and recorded images will be taken by HSI for promotional purposes. I agree that photographs and recorded images of myself, my handlers/staff and associated participants may be taken at the event(s) and I accept that these images will be used in studbook publications, both print media and online media sources for the promotion of the sector.
